Petty Cash Request (pdf)
Petty cash may be obtained up to 24 hours in advance of the event. Fill out and attach this form to Check Request Form. Please allow 72 hours to process. (Completed forms with receipts/invoices attached can be left in the Treasurer folder in the PTA box in the front office.)

Pricing Approval Request
To calculate selling price and sales tax BEFORE the event takes place. (Completed forms with receipts/invoices attached can be left in the Treasurer folder in the PTA box in the front office.) |

Reimbursement Voucher
For reimbursement of money to a PTA Board Member for items purchased for the PTA. Receipts must be attached. Sales tax WILL NOT be reimbursed. A PTA check cannot be issued unless there are sufficient funds remaining in the committee's budget line item to cover it. (Completed forms with receipts/invoices attached can be left in the Treasurer folder in the PTA box in the front office.)

Volunteer Form
Murphy Middle School PTA Volunteer Opportunities - On occasion grade-level teachers, band, orchestra, choir, athletics, student clubs, etc. may request parent volunteers. To help us contact parents with students in specific groups/activities, it will be helpful to know more about your students and what activities/clubs/events they might be involved in during the current school year. Just print out the form, fill it in, and drop it by the front office to the attention of Pauline Samuel, PTA Volunteers Chairman or email to volunteer at murphypta.org (replace at with @). |
