This page is intended for the use of Murphy PTA Board members only.
- Forms are available in the PTA room at Murphy or can be downloaded below.
- Completed forms with receipts/invoices attached can be left in the Treasurer folder in the PTA room.
- Collection Reports are only available from the PTA room because they are 2-part NCR forms. ALL Collection Reports and monies should be given directly to the Treasurer; not left in folder.
- A PTA check cannot be issued unless there are sufficient funds remaining in the committee's budget line item to cover it.
All documents require a password and Adobe Acrobat Reader (to download a free copy of Adobe Acrobat reader, click on the link in the left column). Please remember that the password is case sensitive. If you forgot the password, you may request it by emailing webmaster@murphypta.org. Be sure to include the name of your board position, and the password will be sent to your PTA email account.
Reimbursement Voucher (pdf)
For reimbursement of money to a PTA Board Member for items purchased for the PTA. Receipts must be attached. Sales tax WILL NOT be reimbursed. |
Plan of Work (pdf)
An outline of the goals for an office or committee for the current year. It includes action steps planned to achieve the goals and a budget, if needed. |
Vendor Check Request (pdf)
To pay a vendor or performer for products or services provided. Invoice must be attached. |
Pricing Approval Request (pdf)
To calculate selling price and sales tax BEFORE the event takes place. |
Check Request (pdf)
To request payment to a person that is not a vendor and not a PTA Board member. Documentation must be attached. |
Collection Report (not available here)
To count and record money received at a PTA event. Available only in the PTA room at Murphy. |
